To all WVU Tech Student-Athletes and their Parents
Summer 2010
Just a reminder, all student-athletes are required to have their yearly pre-participation physical exam before returning to campus in August. Half of the form is a medical history that will need to be complete prior to seeing the physician to perform the physical exam. Please note that the pre-participation physical we have you all complete with your doctor is not the same as the general physical required for Admissions. Also, note that Admissions requires a specific form for recording immunizations, so you will need to take it with you to see the doctor but the Sports Medicine Department does not need this form – please send it to Admissions. (Admissions Immunization Form).
The athletic physical exam form has been changed to add a more detailed history and includes additional tests student-athletes need prior to participation. This form will be the only exam form we will be accepting from student-athletes upon their return this fall. The pre-participation physical exam form can be found on-line under “Forms for Athletes”.
The proof of insurance form is also located on-line under “Forms for Athletes”. Please make sure that a copy of your insurance card (front and back) is sent in with the form.
The Sports Medicine Staff also recommends that all student-athletes have medical insurance. WVU Tech offers Athletic insurance that is a secondary policy; meaning the student-athletes’ primary policy will be filed first in the event of injury. Injuries that occur during official practice and games are the only claim that can be made on the Athletic insurance. If a student-athlete does not have primary insurance, an allowable claim can be made, but there is a $250 deductible that has to be paid by the student-athlete for services rendered. If the student-athlete does not have primary insurance we strongly encourage those athletes to contact Student Life and inquire about insurance coverage that the University has to offer students.
All student-athletes are strongly encouraged to make sure that all of this paperwork is mailed to the Sports Medicine Department prior to August 1, 2010. For your own safety, student-athletes who do not have proper medical documentation and proof of physical exam will not be permitted to participate in team activities until we have the proper documentation on file.
